    • Description of the Incident: A clear and concise account of what occurred.
    • Actions Taken: Steps that were immediately implemented in response.
  • Collect Evidence:
    If applicable, secure any physical evidence (e.g., unauthorized materials or defective test books) to accompany the irregularity report.

3. Submission

  • Coordinator Review:
    Test coordinators should review all IRs for completeness and accuracy before submission.

  • Submit to the College Board:
